Vpliv plazmina in plazminogena na ovulacijo

Na Ginetološki kliniki v Ljubljani, Klinicni center v Ljubljani, so leta 1992/1993 uvedli preventivno folikularno punkcijo 10-12 ur po administraciji humanega horijevega gonadotropina (hCG). S tem postopkom že od takrat uspešno preprecujejo sindrom hude ovarijske hiperstimulacije (OHSS): 36 ur po indukciji ovulacije s hCG pa aspiritamo zrele oocite, ki jih nato uporabimo v in vitro fertilizaciji. Ker predstavlja OHSS življenjsko nevarnost za pacientke ( Že v 0,5 -2 % se pojavi v hudi obliki.) poskušamo bolje razumeti njegovo patogenezo. Boljše razumevanje bi nam pomagalo pri preprecitvi tega zapleta. Namen študije je ugotoviti prisotnost in razliko v koncentracijah plazminogena ter indirektno tudi plazmina 12 in 36 ur po indukciji ovulacije s hCG. V študijo je bilo vkljuceno 23 folikularnih tekocin žensk, ki so se zdravile zaradi neplodnosti ( starost maj kot 35 let ) na Ginekološki kliniki v Ljubljani.
Plazminogen smo merili po metodi Berichrom plazminogen ( Behring, Marburg-Lahn, Nemcija). Rezultati
-so pokazali prisotnost plazminogena v folikularni tekocini,
-niso pa pokazali razlike v koncentraciji plazminogena 12 in 36 ur po indukciji ovulacije s hCG.
Glede na rezultat lahko sklepamo, da pri inducirani ovulaciji s hCG plazminogen/ plazminski sistem nima tako pomembne funkcije, kot smo domnevali.

Influence of plasmin and plasminogen on ovulation

In department of obstetrics, Ljubljana, medical Center introduced in the year 1992/1993 preventive follicular punction (ETFA )10 -12 hours after administration of human chorionic gonadotrophin ( hCG). With that procedure is successfully prevented severe ovarian hyperstimulation syndrome (OHSS).
36 hours after induction of ovulation with hCG they aspirate mature oocits. These are afterwards used in in vitro fertilisation. Because OHSS is life treatening for patients ( severe form occurs in 0,5 -2 %) we are trying to understand its pathogenesis better. With this we could help in prevention in this study.
The aim of the study was to find out the presence and difference in concentration of plasminogen and indirectly also plasmin 12 and 36 hours after induction with hCG. 23 follicular fluids involved in our study, from women, who were treated because of infertility (ages less then 35 years) in Gynaecological department of Medical center in Ljubljana.
Plasminogen was measured with Berichrom ( Behring, Marburg -Lahn, Germany). The following observations were found :
-the presence of plasmogen in follicular fluid was determined,
-there was no significant difference between concentration of plasminogen 12 and 36 hours after induction of ovulation with hCG.
With regard to result we can assume that in case of induced ovulation with hCG plasmogen/plasmin system is not that important as we suspected.
